Physical therapy is used to improve a patient’s physical functions through physical examination, diagnosis, prognosis, physical intervention, rehabilitation and patient education.
Population Health Physical Therapy is the referral of individuals with chronic disease or health risk into evidence-based programs through an evaluation and exercise prescription by a physical therapist for safe and effective physical movement sustainable for a lifetime. We look to transition these individuals into medically-integrated health and wellness facilities within local communities. Price $199.59 (2- Units of Physical Therapy Treatment / 45-minutes)
Have you ALREADY SEEN THIS THERAPIST for an initial evaluation of your problems and still have issues or need follow up care? While therapists do their best to treat your problem on your first visit additional treatments are often needed. This package is for follow up care of your problem that is generally less complex than your initial evaluation. PURCHASE THIS PACKAGE ONLY IF YOU HAVE ALREADY SEEN THIS THERAPIST FOR THE SAME PROBLEM AND NEED FOLLOW UP CARE.
