Registered Nurse

Core Health Partners model of clinic to community service offers a Registered Nurse to help navigate and encourage patients to make healthy choices; teaching people about wellness is the primary focus of nurse coaches. Our Nurses oftentimes work with individuals who have chronic health issues; such as cardiovascular disease and diabetes. The Core Health Partner Nurse evaluates the patient’s needs and conditions and working alongside our other healthcare professionals they can design and adopt appropriate dietary changes, exercise limitations, while measuring and monitoring these adjustments to help guide the patient to live a healthy life. The goal is for patients to thrive through our mind-body and spirit approach to patient care. Many who are suffering through a life with a chronic illness are able to improve their well-being through the education of medications prescribed by prescriptions and/ or lifestyle changes provided through our Nursing services. The Nurse also teaches the patient about pain management and how to recognize symptoms of complications. The Core Health Partners Nurse maintains a flow of communication between the patient, caregiver, and physician to facilitate proper care for the patient.
